Consent
The first step in enrolling a participant in the STOP Program is to obtain their consent. Without a signed consent form, the participant is not eligible to enroll in the program and receive free NRT. Key points to remember include the following:
- Consent must be obtained through a paper form with information and signatures completed using a pen.
- If the participant is under the age of 16, both the participant and their parent/legal guardian are required to sign the form.
- The date which the participant signs the consent form is their official enrollment date.
Consent Procedure
The consent procedure includes the following steps:
- The participant must read and understand the study purpose, procedures, risk and benefits of participating, and be made aware of their ability to withdraw at any time without penalty.
- The participant can ask questions and it is your responsibility to respond.
- The participant will sign and date the form on the day of the first visit.
- You will sign and date the form, which must match the date participant signed.
- You will provide the participant with a copy of the consent form.
- Once consent is obtained, you may proceed to filling out the other study questionnaires and dispense NRT.
