Consent

The first step in enrolling a participant in the STOP Program is to obtain their consent. Without a signed consent form, the participant is not eligible to enroll in the program and receive free NRT. Key points to remember include the following:

  • Consent must be obtained through a paper form with information and signatures completed using a pen.
  • If the participant is under the age of 16, both the participant and their parent/legal guardian are required to sign the form.
  • The date which the participant signs the consent form is their official enrollment date.

Consent Procedure

The consent procedure includes the following steps:

  1. The participant must read and understand the study purpose, procedures, risk and benefits of participating, and be made aware of their ability to withdraw at any time without penalty.
  2. The participant can ask questions and it is your responsibility to respond.
  3. The participant will sign and date the form on the day of the first visit.
  4. You will sign and date the form, which must match the date participant signed.
  5. You will provide the participant with a copy of the consent form.
  6. Once consent is obtained, you may proceed to filling out the other study questionnaires and dispense NRT.
