Summary of Bob’s Case:

  • Bob has been working with you for a few weeks now, since his most recent hospitalization, and he has never missed an appointment.
  • Bob has a long daily smoking history of over 40 years. He used to smoke more than 1 pack per day (20 or more cigarettes per day) but he has been quit since his last exacerbation/ hospitalization and using NRT - wearing 2 nicotine patches (21mg + 14mg) per day.
  • Bob’s COPD medication is at the maximum dose, and except for attending your appointments and other medical appointments, he has been fairly house bound, very dependent upon his wife, increasingly anxious and depressed.
  • You and his other health care professionals have recommended a referral to a Pulmonary Rehab Program but Bob has declined this referral.
  • At your last visit, even though Bob was still adamant about not attending the Pulmonary Rehab Program, he did see the value around increasing his physical activity level.
  • At Bob’s second visit, he had reported that he had begun to increase his physical activity level at home, and he seemed pleased about this change and he seemed very motivated to further increase his activity level at home.
  • He reported that his wife was also supportive of this plan so that they could start walking together in their apartment building.

Practitioner: “Hi Bob, it’s good to see you. How are things since we last met?”

Bob: Pretty much the same, maybe even a little worse. Still not leaving the apartment except when I have to attend appointments. Though I’m actually glad to be here today. Just to get away from my wife for a little while. She has been on my case all week, nagging me about my health, and complaining about every little thing. I told her not to even bother coming with me today because I need a break from her so called “support”.
